1. Innovation Name  (Name of project, policy, practice, strategy, etc.)

 

2. Innovation Type  (Select the one that best describes the area of your innovation)

  • Assessment/Placement
  • Instruction/Delivery
  • Accountability/Continuous Improvement
  • Student Supports

 

3. Snapshot of your innovation. What is it, who does it serve, what does it do? What are its intended outcomes?

 

4. What do/did you hope to achieve with this innovation and why?

 

5. Tell the Story of your Innovation  What propelled the change? What are the key features? How many students, classes, or institutions are involved? How long has it been in place? Who was involved in its development? How is it funded?

 

6. What evidence do you have that your innovation is having a positive impact? To what do you attribute this impact? An overview of how the innovation has been evaluated and the results, using bullet points when possible, and attach any documentation in this box or Question 8.

 

7. What might be the barriers for others to adopt/adapt your innovation?

 

8. Upload  any additional information about your innovation. Including links to or documents related to project sites, policies, evaluations, data, papers, articles, research that supports your innovation, etc.
